填寫表格、上傳證件並完成電子簽名。送出後系統會安全儲存紀錄，並自動下載三頁 PDF。 Complete the form, upload the required documents, and sign electronically. After submission, the record is securely saved and the three-page PDF downloads automatically.

除 Middle Name、House No.、Barangay 與 Zipcode 外，其餘欄位皆為必填；Beneficiary 1 必須完整填寫。
All fields are required except Middle Name, House No., Barangay, and Zipcode. Beneficiary 1 must be completed.

Passport and ARC Copies

請上傳清楚、完整且四角可見的影像。護照為 PDF 第 2 頁，ARC 正反面為第 3 頁。 Upload clear, complete images with all four corners visible. The passport is page 2; ARC front and back are page 3.

簽名照片

Signature Photo of Worker

簽名照片必須與護照簽名一致。 The photographed signature must match your passport signature.
拍照／選圖後先手動拖曳、放大並確認裁剪位置，再自動去背並轉成黑色透明 PNG；灰白棋盤格代表透明區域。 Crop and position the photo first, then it will be cleaned and converted to a black transparent PNG.
